This is a web-based software package that can be used to survey students about course related assessment/evaluation issues. It also contains step-by-step instructions and suggestions on how to customize it with specific questions for a given objective and for other surveys. The file contains a general description of functions and manual in MS Word for the general reader as well as the UNIX based application program and installation instructions.
